PTNet
PTNet is a preferred network of outpatient physical therapy providers who specialize in the treatment of injured workers. PTNet is designed exclusively to maximize efficient access to specialty providers and improve outcomes while increasing the level of savings to our customers, PTNet enables our clients to significantly reduce the cost of rehabilitation services without disrupting the normal workflow of their claims process.
For the convenience of our customers, specially trained PTNet Referral Coordinators will schedule each initial appointment We will notify the referring party with confirmation of the date, place and time of the initial evaluation, as well as verify patient compliance with their first visit.
If you are enrolled in the PTNet program, the Referral and Authorization Form is accessible on line. You may complete the form on-line then E:mail it in or print and fax to the number listed below. One of our PTNet Referral coordinators will notify you within 48 hours with the patient’s appointment time, date and location.
